Steps to verifying that you meet the requirements are as follows:
Step 1. Qualifying conditions:
Do I have a cancer diagnosis? If yes, proceed to Step 2.
Do I have an epilepsy diagnosis? If yes, proceed to Step 2.
Do I have a glaucoma diagnosis? If yes, proceed to Step 2.
Do I have intractable migraines unresponsive to other treatment? If yes, proceed to Step 2.
Do I have a diagnosis of a chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, Parkinson’s disease, and Tourette’s syndrome? If yes, proceed to Step 2.
Do I have a diagnosis of a debilitating psychiatric disorder, including, but not limited to, post-traumatic stress disorder, if diagnosed by a state licensed psychiatrist? If yes, proceed to Step 2.
Do I have a diagnosis of human immunodeficiency virus (HIV) or acquired immune deficiency syndrome (AIDS)? If yes, proceed to Step 2.
Do I have a diagnosis of a chronic medical condition that is normally treated with a prescription medication that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication? If yes, proceed to Step 2.
Do I have a diagnosis of any terminal illness? If yes, proceed to Step 2.
Do I have a diagnosis, in the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C, amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia, and wasting syndrome? If yes, proceed to Step 2.
Step 2. Gather your medical records/documentation*:
At this point, you should gather your official medical documents related to your qualifying medical condition (listed above). Medical records/documentation may include but are not limited to:
a. Hospital discharge records;
b. Clinic/doctor visit records;
c. Prescription/medicine fill/refill history records from pharmacy; or
d. Any other official documentation from a licensed healthcare provider.
